We are excited to share our survey on the My Health Care Wishes document we co-created with a group of self-advocates with lived experience. My Health Care Wishes supports people to make decisions about their health. It also supports people to have their choices understood and respected when they access health services.
This survey will explore how people, their supporters, advocates, and health professionals think about shared medical decision-making. The results of the evaluation will be used to improve the document and to help individuals, health professionals, and disability support professionals to better understand the needs of people with intellectual disabilities.
